a. The Client acknowledges and agrees that a service charge of $25.00 shall be paid to Amor Healthy Lifestyle (payable to Jennifer Amor) in the event of either a returned payment due to insufficient funds or a missed payment or in collection of past obligations to Amor Healthy Lifestyle, including court costs and reasonable solicitors fees.

4. Rules and Regulations

a. Proper attire must be worn during work out sessions and group classes. Bare feet, sleeveless t-shifts, t-shifts, tank tops, leotards, leggings and tracksuits can all be considered appropriate attire. Attire such as flip-flops, sandals, loose fitting shoes, are deemed inappropriate. Outerwear (sweaters etc.) can be worn at the Client’s discretion.

b. The Client agrees to submit to and complete the Par-Q, or any other necessary medical documents.

c. Amor Healthy Lifestyle reserves the right to amend or add to these rules and regulations and to adopt new rules as it may deem necessary.

5. Indemnity

a. The participant/ client expressly acknowledges that he/she will be engaging in physical exercise and activities while attending Amor Healthy Lifestyle’s program. The Participant/ Client hereby states that he/ she is and will

                Voluntarily be participating in these physical exercise and activities and the Participant/ Client hereby assumes all risks of injury which might result from these physical exercises and activities. The Participant/ Client hereby waives and releases all claims that he/she now has or may have against Amor Healthy Lifestyle, its employees or agents for injury sustained by the Participant/ Client as a result of these physical exercise and activities. The Participant/ client hereby acknowledges that he/she has carefully read this waiver and release and fully understands that it is a release of liability of Amor Healthy Lifestyle and aggress that such a waiver and release is reasonable and proper based on the nature of Amor Healthy Lifestyle’s business. In the event of an injury, the Participant/ client must immediately notify Amor Healthy Lifestyle and provide full written particulars of the injury and the accident upon being requested to do so. The Participant/ client hereby release and hold harmless Amor Healthy Lifestyle, their agents, officers and employees and any affiliated companies from any liability with respect to injury to the Participant/ client and their property, arising out of or connected with the Participant/ client’s physical exercises and activities. It is stated and agreed that this term shall apply for all facilities by and for Amor Healthy Lifestyle related fitness activities and applies to in home personal training as well.

b. The Participant/ client hereby waives and releases Amor Healthy Lifestyle from any claims that he/she may have against Amor Healthy Lifestyle with respect to any loss or theft of personal property occurring during a training session or class.

c. The Participant/ client hereby warrants and represents that he/she is in good physical condition and that he or she has no liability, impairment preventing him/ her from engaging in active or passive exercise that will be detrimental or injurious to his/her health, safety, comfort and physical condition or that of others. The Participant/ client hereby represents that he /she will not use any of Amor Healthy Lifestyle’s other facilities or services while having any open cuts, abrasions, open sores, infections, illnesses or any other physical impairments that could be aggravated by use of the facilities, or engaging in the fitness program. The management of Amor Healthy Lifestyle’s facilities or services shall have the right to make a final binding decision determination in this regard.
